{"id":17164,"date":"2020-11-24T14:10:37","date_gmt":"2020-11-24T14:10:37","guid":{"rendered":"https:\/\/www.sygnaturediscovery.com\/blog\/abuse-liability-assessments-how-to-get-what-you-want\/"},"modified":"2020-11-24T14:10:37","modified_gmt":"2020-11-24T14:10:37","slug":"abuse-liability-assessments-how-to-get-what-you-want","status":"publish","type":"blog","link":"https:\/\/www.sygnaturediscovery.com\/fr\/blog\/abuse-liability-assessments-how-to-get-what-you-want\/","title":{"rendered":"Abuse liability assessments\u2026 how to get what you want"},"content":{"rendered":"<p><span style=\"color: #808080;\"><strong><em>Why the beginning may be a very bad place to start<\/em><\/strong><\/span><\/p>\n<p><span style=\"color: #26bdef;\">By Andy Mead, DPhil \u2013 Director and Head of Head of Drug Abuse and Substance Use Disorders<\/span><\/p>\n<p>Over the years, I\u2019ve seen numerous examples where the sponsor has been left disappointed or frustrated at the drug scheduling decision made on their recently approved drug.<\/p>\n<p>Typically, these discussions involve statements such as <em>\u201c\u2026but they clearly didn\u2019t understand the data<\/em>\u201d or <em>\u201c\u2026the problem is that the regulators don\u2019t understand the scienc<\/em>e\u201d.\u00a0 Statements of opinion, that often have little factual basis. So why does this happen? <strong>Why don\u2019t agencies and sponsors always see the same information in the same way?<\/strong><\/p>\n<p><a href=\"https:\/\/www.sygnaturediscovery.com\/wp-content\/uploads\/2025\/11\/pills-2.jpg\"><img loading=\"lazy\" decoding=\"async\" class=\"alignnone wp-image-9684 size-full\" src=\"https:\/\/www.sygnaturediscovery.com\/wp-content\/uploads\/2025\/11\/pills-2.jpg\" alt=\"\" width=\"1000\" height=\"481\"><\/a><\/p>\n<p>There is no doubt that the regulators have a difficult task \u2013 to make a call on whether to recommend restrictions on access and distribution of a novel therapeutic in order to mitigate risk of abuse and misuse of the drug outside of its therapeutic setting. This decision is typically informed by a relatively limited data set of controlled non-clinical and clinical studies and perhaps some limited epidemiological data. And all of this takes place in the context of shifting societal perception regarding drug abuse and changing trends in actual drug abuse.<\/p>\n<p>A sponsor can\u2019t control this decision. <strong>What they can do, however, is influence this decision by generating the right data in the right way<\/strong> to ensure that the regulators\u2019 have all the information they need in order to increase the chances of getting this decision right.<\/p>\n<p>So, how exactly can the sponsor do that?<\/p>\n<p>\u00a0<\/p>\n<h3 style=\"padding-left: 40px;\"><span style=\"color: #006aab;\"><strong>1. Start with the decision in mind<\/strong><\/span><\/h3>\n<p style=\"padding-left: 40px;\"><strong><a href=\"https:\/\/www.sygnaturediscovery.com\/wp-content\/uploads\/2025\/11\/decision-in-mind-2b.png\"><img decoding=\"async\" class=\"size-full wp-image-9687 alignright\" src=\"https:\/\/www.sygnaturediscovery.com\/wp-content\/uploads\/2025\/11\/decision-in-mind-2b.png\" alt=\"Keep the decision in mind\"><\/a>The most important step is to start at the very end of the process \u2013 a concept popularised by Stephen Covey in <em>\u2018The 7 Habits of Highly Effective People\u2019<\/em>.<\/strong><\/p>\n<p style=\"padding-left: 40px;\">Think about the preconceptions that the decision makers (the regulatory agencies) will have. A novel opioid agonist may well be viewed with significant caution given the very public opioid crisis that has swept the US. Similarly, a novel mechanism for ADHD will likely be compared to approved stimulant medications. Preconceptions will exist based on the mechanism of action, the therapeutic area, the nature of adverse events observed in clinical studies and much more.<\/p>\n<p style=\"padding-left: 40px;\">What data would it take to change those preconceptions? If it were you, what data would it take to change your mind? This is not about influencing inappropriately. At the end of the day, the most important thing for both sponsor and agency is that we end up with the right decision. This is about increasing the probability that we end up with that right decision.<\/p>\n<p style=\"padding-left: 40px;\">What are the questions the agency will ask of the data? How do you generate data that <em>specifically<\/em> addresses those questions? Only with this level of forethought can you help the decision maker come to the same conclusion that you have based on the data.<\/p>\n<p style=\"padding-left: 40px;\">Which leads me to\u2026<\/p>\n<p>\u00a0<\/p>\n<h3 style=\"padding-left: 40px;\"><span style=\"color: #006aab;\"><strong>2. Generate the right data<\/strong><\/span><\/h3>\n<p style=\"padding-left: 40px;\"><strong><a href=\"https:\/\/www.sygnaturediscovery.com\/wp-content\/uploads\/2025\/11\/data-1.png\"><img decoding=\"async\" class=\"size-full wp-image-9686 alignright\" src=\"https:\/\/www.sygnaturediscovery.com\/wp-content\/uploads\/2025\/11\/data-1.png\" alt=\"\"><\/a>It is critical that studies are designed to the highest level of scientific quality.<\/strong><\/p>\n<p style=\"padding-left: 40px;\">Within a single study, regardless of the nature of that study, there is a clear objective, with a design specifically tailored to achieving it. Focus on this is critical.<\/p>\n<p style=\"padding-left: 40px;\">To take one example: it is possible to design a drug self-administration study in many ways. What schedule of reinforcement should I apply? What training drug should I use? Should I include reference comparators? If so, how many, and which ones?<\/p>\n<p style=\"padding-left: 40px;\">The relevant regulatory guidance provides insight into the framework that we need to work within, but it doesn\u2019t (and shouldn\u2019t) provide the detailed answers to these questions. This is your opportunity to tailor specific studies to achieve specific objectives. Each study then feeds into the broader overall objective in terms of getting the right decision for your drug.<\/p>\n<p style=\"padding-left: 40px;\">So, you\u2019ve generated the data. What next?<\/p>\n<p>\u00a0<\/p>\n<h3 style=\"padding-left: 40px;\"><span style=\"color: #006aab;\"><strong>3. Think about how you visualize and analyse your data <\/strong><\/span><\/h3>\n<p style=\"padding-left: 40px;\"><strong><a href=\"https:\/\/www.sygnaturediscovery.com\/wp-content\/uploads\/2025\/11\/Analyse.png\"><img decoding=\"async\" class=\"size-full wp-image-9685 alignright\" src=\"https:\/\/www.sygnaturediscovery.com\/wp-content\/uploads\/2025\/11\/Analyse.png\" alt=\"\"><\/a>When designing studies, it can be too easy to think that the majority of the job is done once the study plan is agreed and the study is underway.<\/strong><\/p>\n<p style=\"padding-left: 40px;\">How often do you think, at the study design stage, how you\u2019re going to present your data? What data is going to be front and foremost? How will it be presented? Why are you presenting it that way?<\/p>\n<p style=\"padding-left: 40px;\">Again, this needs careful consideration, and alignment with the overall objectives. You know who you want to influence. You now know, relative to likely preconceptions, how they may need convincing. Did you adjust your data visualisation and analysis to align with this? So, what is the data visualisation that is most likely to convince them?<\/p>\n<p style=\"padding-left: 40px;\">The same is true for analysis. How you choose to analyse the data should be clearly aligned to the story you are looking to tell. There is no single right way to visualise and analyse data. There is however a right way when it comes to aligning with the overall strategy to achieve the top-level objective of telling a compelling story.<\/p>\n<p>\u00a0<\/p>\n<h5><em><strong>What does this all mean? <\/strong><\/em><\/h5>\n<p>Well, put simply, it might be time to flip things on their head when designing your abuse liability package. Instead of starting with the regulatory guidance, and designing a package of studies that simply achieves the requirements of that guidance, start at the end of the process, and remember the following key points:<\/p>\n<p>\u00a0<\/p>\n<ul>\n<li><strong><span style=\"color: #26bdef;\">Put yourself in the mind of the decision maker.<\/span> What will they be thinking? What are their motivations? What are their unconscious biases likely to be?<\/strong><\/li>\n<li><strong><span style=\"color: #26bdef;\">How do I tell a story that helps the decision maker come to the right conclusion?<\/span> Have I designed the right studies to help them do that? Have I analysed and presented the data in the best way to help them form a position?<\/strong><\/li>\n<li><strong><span style=\"color: #26bdef;\">Strong science.<\/span> It\u2019s very difficult to argue with quality science. Sometimes it\u2019s right to do things differently to achieve your outcome, but you must ensure it can be defended on a strong scientific basis. If not \u2026 good luck!<\/strong><\/li>\n<li><strong><span style=\"color: #26bdef;\">Don\u2019t forget the regulatory guidance.<\/span> It is of course important that we work within this framework. But the guidance is just that \u2026 guidance. It doesn\u2019t tell you what to do; it presents the opportunity for you to do the right thing for your situation. The onus is very much on you to present the right information in the right way.<\/strong><\/li>\n<li><strong><span style=\"color: #26bdef;\">Ultimately, our job is to make the decision simple for the agencies.<\/span> This is most commonly achieved by presenting a clear and defendable position, with studies specifically designed to remove uncertainty. The more uncertainty there is, the greater the likelihood that the decision won\u2019t align with what you want.<\/strong><\/li>\n<\/ul>\n<p>\u00a0<\/p>\n<p>This article only begins to touch on concepts important in designing an appropriate abuse liability package.
